Lebanon Anesthesia Associates (LAA) accepts all forms of health insurance and will submit a health claim form to the health plan provided by you or the hospital/surgery center. Once the health plan has paid, you will be billed for any balance that your health plan applied to your deductible or co-insurance. You will also be billed if you have no insurance coverage or if your health plan deems you not eligible for benefits. It is your responsibility to settle any accounts not covered by your health plan as soon as possible.
We accept the following methods of payment for our services:

Anesthesia Service Billing
LAA bills on an independent basis for our professional services. Charges are based on the surgical procedure, the anesthesia time required for the surgery and the patient’s health status. The hospital or facility will charge for equipment, drugs and supplies used by our physicians and certified registered nurse anesthetists (CRNAs). Questions regarding the facility charges should be directed to the facility’s Patient Service Department.
Split Billed Services
Medicare and other health plans require LAA to submit two claims for our services, one for the anesthesiologist and one for the CRNA. Please note that you are not being billed twice.
LAA is happy to provide an estimate of the charges that will be incurred for your upcoming procedure. However, we cannot guarantee the actual total charge because anesthesia services are billed, in part, on the length of time our anesthesia providers spend with you before, during and after your surgery. Thus, the actual charges for your surgery cannot be ultimately determined until after the surgical event. To receive an estimate please contact us and include the charge code (CPT) code for your upcoming procedure.
